- Access and obtain copies of your medical records
- Request corrections to inaccurate medical records
- Obtain a written copy of this notice
- Request restrictions on certain disclosures
- Receive an accounting of disclosures made
When Authorization Is Required
Outside of the situations listed above, HRC Fertility will obtain your written authorization before using or disclosing your health information, including for marketing purposes and the sale of information. You may revoke any authorization you have given at any time.
How to File a Complaint
If you believe your HIPAA privacy rights have been violated, you may contact our Privacy Officer or file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights (HHS OCR). You will not be retaliated against for filing a complaint.
